For Friday Evening Games Held in Medfield Only

You will be able to Check-In at the Wheelock Field Complex. 

ONLY teams playing Friday night at Medfield will be allowed to Check-In there beginning at 4pm.  Teams playing at 5 check-in first and teams playing at 6pm check-in after all the early games teams are checked in.

IF Your First Tournament Game is on Saturday Afternoon

For teams who are not playing their first game until late on Saturday you can register at the information booth at DeFazio Park after 12 PM. Please note Check-In will be unavailable between 11 AM and 12 PM on Saturday the 29th as we switch over from the Sheraton to DeFazio Park. Please plan accordingly and we strongly encourage all teams to register Friday night or Saturday morning at the Sheraton.

 

IMPORTANT INFORMATION FOR ALL COACHES / MANAGERS
When Checking-In you must confirm your on-site phone contact that you entered during the on-line registration. This contact will be used to communicate with your team in the event of a schedule change or other critical situations that arise. It will be up to this contact to then contact the rest of your team to alert them to the changes. This contact number should be available 24/7 throughout the tournament.

You must also arrange to have access to your e-mail and the internet during the tournament. All tournament hotels will have high-speed or dial-up connections available. Updates to schedules and other general changes to the tournament will be communicated via e-mail and the tournament website so access to these facilities are mandatory.

 

Team Check-in Requirements

This year in an effort to have many check-in requirements done prior to your arrival and avoid long lines at the Sheraton on Friday night, we have developed the tournament Fast Lane Check-In.  You MUST follow the procedures below to make this happen:


  1. You must enter your roster ONLINE in the Got Soccer system by May 1st  including jersey numbers.
  2. You must have all your team paperwork completed and submitted TOGETHER.  All NMDT medical release forms must be completed and signed by the parent.  No forms other than the official Needham Memorial Day Tournament medical release form will be accepted.  It is available on the tournament website Forms Page in PDF and word doc format.  Scan completed forms to a PDF or JPEG file and email them to the nmdtpaperwork@needhamsoccer.com —alphabetical order by last name please. Emails with all necessary paperwork must be received by Tuesday, May 25th 6pm. The subject line of the email must contain your team name, age group and gender – i.e. Soccer Club Eagles Gu12.
  3. If you are scanning and emailing your medical release forms to us, DO NOT SEND THEM IN THE MAIL.  We do not need to duplicate efforts!  Bring them with you and have them at all games in case you need them.
  4. A scanned copy of your current approved league roster must also be emailed to nmdtpaperwork@needhamsoccer.com
  5. If you are outside of Region 1, you will need to submit your permission to travel form via email.
  6. Managers or coaches must have copies of all player birth certificates (or some proof of age—copy of approved league roster is ok) and passcards (if passcards are required by your league) in your possession at every tournament game (in case we need to check you further or if we have a red card issue!).  You must also have a copy of all the medical release forms with you at every game.
  7. IF you have completed all the appropriate paperwork described in these instructions, we will email you a Fast Lane pass (PDF) the day before the tournament which you will need to print and produce at Check-In.  You are still required to go to the Sheraton on Friday night; however you will be directed to a location on the main level of the hotel and will not be required to go downstairs to check-in.
  8. You will need to provide an on-site phone number available 24/7 for your team when you check-in.
If you choose not to use the Fast Lane Check-in, please read all the instructions below for Tournament Check-in at the Sheraton Needham Hotel 3-9pm on Friday, May 28th

Player Identification: Proof of player’s age for all rostered players; can be official player passcards, birth certificates or driver’s licenses.  If birth certificates are used, please highlight DOB and player name and alphabetize the forms by player last name.  If passcards, please have all cards in alphabetical order.
Roster: ALL rosters MUST be entered online through the GotSoccer.com system by MAY 1ST .  You must also bring a copy of your league roster to Team check-in.  If you do not have a certified copy from your league, you may use a typed player list.
Guest Players: Anyone using a guest player in the Needham tournament MUST provide a copy of that player’s birth certificate and a current State Membership Form and/or player passcard.  Needham Soccer Club (NSC) Teams with guest players MUST contact the NSC soccer office Admin@needhamsoccer.com
Permission to Travel paperwork: Teams from Region I states do not need Permission to Travel paperwork.  Permission to Travel forms must be provided for all other teams outside of Region 1 at Team check-in.
Foreign Teams: Players must present passports at registration or, if from a nation that the United States does not require a passport, proof of entry into the United States that is required by the United States. Teams are required to have and present player picture identification cards. Tournament rules require that the team must have a completed form from its Provincial or National Association approving the team’s participation in the tournament.
Medical Release Forms: Needham Memorial Day Tournament Medical Release/Liability forms ARE REQUIRED FOR EVERY PLAYER.  You are required to mail them to the tournament by April 30th if you have chosen NOT to participate in the Fast Lane Check-in.  You are also REQUIRED to have copies with you at Team Check-in (if you did not previously email them) and at EVERY game.  Please DO NOT SEND THEM PRIORITY MAIL AS IT DELAYS OUR RECEIVING THEM.  If you have not mailed them by the deadline, and/or have late roster additions or guest players on your roster you must bring copies with you to Team check-in. No player will be allowed to participate in the tournament unless they have a signed NMDT medical release form.

No medical release forms other than the official Needham form will be accepted.
Teams who show up without the proper medical release forms will be prevented from playing until the required forms are produced. This may result in a forfeit being issued for any games missed.


In an effort to minimize emails prior to and during the tournament due to our size, WE REQUIRE ALL TEAM MANAGERS TO PLEASE READ ALL INFORMATION BELOW THOROUGHLY.  We also require that all coaches and team managers read our tournament Procedures and Rules, SPECIFICALLY OUR WEATHER POLICY.  BEFORE SENDING ANY EMAILS PLEASE GO TO OUR FAQ PAGE ON OUR WEBSITE; answers such as “why we mandate the use of our medical release form” are posted there.  Please discourage your team parents from emailing the tournament (unless urgent or injury related).  Encourage them to go through coaches and team managers for information or to use our website.  Thank you.
